It is with mixed emotions that I announce my retirement as of November 30, 2020. The office is now officially closed. Many of you received my letter explaining the process for request of medical records and including some recommendations for your ongoing care. I have recently updated the website with further details that I hope will assist you with this transition. Medical records requests will be coordinated through Dr. Jeannine Stein's office. A signed release form will be required (any doctor's form is acceptable, or you may download/print a form from this website) and can be mailed, faxed, or delivered to her office. Contact information is as follows:
