Tooele County Health Department Community Health Workers (CHW) are state certified to help link residents with the health and social resources they need to live their healthiest lives. CHWs can help you discover and navigate resources, including access to healthy food, nutrition education, active living, aging services, transportation, COVID-19 vaccines and testing, and more!
