Instructions: Download the REPAIR FORM. Include a COMPLETED copy of the Repair Form in the box. Properly package your equipment. Ship equipment to our Lebanon, TN location (address on form). (Via UPS, FedEx, etc.) If you check the box on the form, we will contact you with an estimate. Per your approval, we will repair your equipment and return it to the address provided on the Repair Form. We will provide a 90-day warranty. Note: Our Labor rate is $100/hr.
